If you're interested in a New Patient Appointment with Dr Earthman please fill out the information below and it will be sent directly to Dr Earthman's office staff. After you submit your request, our appointment scheduler will contact you as soon as possible.   This is typically within three business days.  Please keep in mind that we are not open on Fridays, Saturdays, Sundays, or most Federal Holidays.
